| Q: Where is CNS Registration? |
| | The CNS Registration Area is located in Level One Lobby of the Moscone West Convention Center. |

| Q: Where can I talk to a CNS staff member regarding special issues or problems with my registration? |
| | In the CNS Registration Area - Level One Lobby of the Moscone West Convention Center there will be a Registration Assistance Booth. CNS staff members will be available during registration hours to answer any questions. |

| Q: Where can I purchase additional tickets? |
| | In the CNS Registration Area at the Moscone Convention Center, there will be an On Site Registration Area for additional ticket purchases. |
|
| Q: If I registered in advance, where can I pick up my materials (which include badge, tickets and program book)? |
| | In the Advance Registration Booth in the CNS Registration Area at the Moscone West Convention Center; the first letter of registrants’ last name will mark the booths. The registrant can pick up all registration materials at this booth. |
|
| Q: If I am not pre-registered for the meeting and want to register, where do I go? |
| | Proceed to the CNS Registration Area - Level One Lobby of the Moscone West Convention Center. Medical Registrants should go to the ON-SITE REGISTRATION booths; exhibitors should go to the EXHIBITOR REGISTRATION booth. |

| Q: Can I pick up my friend/colleague/relatives registration packet? |
| | No. Only the primary registrant can pick up packets. Spouses cannot pick up the packet, nor can friends, colleagues, or relatives. CME credit is granted based on whether the primary registrant claims the packet and therefore only the primary registrant may take it from the registration desk. |

| Q: I did not receive a pre-registration packet and now the session I want is full. Can I get a ticket? |
| | Pre-registration packets are mailed to everyone residing in the CNS Neurosurgical database. This includes all members, as well as residents and non-members who have notified the CNS office of their interest in the meeting. The Member Services personnel at CNS Member Services Center, booth #337 in the Exhibit Hall, can tell you whether or not your name resides in the database and add you to the database at your request.
Seats are limited in luncheon seminars, practical courses, social events and tours. Tickets for those events are sold at the Registration Assistance Booth, on a first come/first served basis. Once sold out, no additional tickets can be sold. However, there are often other interesting courses available at the requested time. Please check your program and the monitors for course selection and availability. |

| Q: My confirmation of registration indicates I have overpaid and have a refund due. How and when can I get my money back? |
| | Refund checks are mailed approximately four to six weeks after the annual meeting. However, while you’re at the meeting you may use your credit balance to purchase tickets to other events, both scientific and social. |
|
| Q: My registration badge is not correct. Can it be fixed? |
| | Yes, see the registration personnel at the REGISTRATION ASSISTANCE booth, located in the CNS Registration Area - Level One Lobby of the Moscone West Convention Center. They can fix any medical attendee's badge or their guest's badge. |
|
| Q: I don’t want to attend some of the scientific programs. Can I go on a tour instead? |
| | No. It is against the CNS policy for any medical registrant to attend functions that conflict with the scientific sessions. In order to uphold the standards of the Accreditation Council for Continuing Medical Education(ACCME), medical registrants cannot attend events which conflict. |

| Q: Can Medical Attendees attend an Optional Evening Event? |
| | Optional Evening Events are available to all registered attendees and our exhibit partners.
CNS Social Activities are also open to all registered attendees and are included with each registration. |
|
| Q: Are children allowed to attend the CNS Annual Meeting? |
| | Children over the age of 12 may register as a guest at the Guest Registration fee of $115. This will allow full participation in Auxiliary Program Activities, access to the Spouse Hospitality Suite and Opening Reception tickets. Please note that children under the age of 18 are not allowed in the Exhibit Hall. |

| Q: Is there day care available onsite? |
| | Should you desire Babysitting services, please contact Core Group (415-567-0568) or American Child Care (415-285-2300). CNS has no control over and assumes no responsibility for the care that is provided through hotels or these services. This information is provided solely to assist participants in identifying possible sources of childcare. |

| Q: Where can I check on the availability of the ‘ticketed’ event (scientific or social)? |
| | Monitors will be placed in the CNS Registration Area - Level One Lobby of the Moscone Convention Center. These monitors scroll all “ticketed” events. The registrant can check these screens to see if any tickets are available for purchase. |
|
| Q: I lost my ticket. Can I get a replacement? |
| | Yes. See the registration personnel at the REGISTRATION ASSISTANCE booth, located in the CNS Registration Area - Level One Lobby at the Moscone West Convention Center. |
|
| Q: Can I get an extra program book for my associate back home? |
| | If there are any extra program books left on Thursday morning, they will be available for your non-registered associates. Come to the registration area located at Hilton San Francisco on Thursday to receive extra program books. |
|
| Q: When and where do I go to set up my poster? |
| | Proceed to the POSTER REGISTRATION booth in the CNS Registration Area – Level One Lobby of the Moscone West Convention Center, which will be open from 12:00 noon – 5:00 pm on Sunday, October 17. Posters will be set in Levels Two and Three Lobbies at the Moscone West Convention Center. Registrants can see the poster desk for exact location of where to display their poster and to get pushpins. |

| Q: When is the early registration cut-off date? |
| | Early registration ends September 15, 2004 at 5:00 p.m. – Add $100 to the registration fee for each category after September 15th. |
|
| Q: What is the cancellation policy? |
| | If requests are received by September 15, 2004, then a full refund is issued. If the request is received by September 22, 2004, then a full refund less a $75 processing fee is issued. After September 22, 2004 absolutely no refunds will be issued, regardless of the cause. No returns or exchange of tickets are allowed on-site. Cancellations requests can be received via email at cns@expoedge.com or via fax at 847-940-2386. |

| Q: Where can I get a receipt for my registration to the CNS Annual Meeting? |
| | If you registered in advance using the online registration service, a confirmation was sent to you with all your registration fees listed.
If you registered onsite, at the time of your registration, the cashier gave you a receipt for your registration fees paid. It lists the registration, practical courses, luncheon seminars and other events, if any, that you signed up for.
Please contact the CNS Headquarters Office if you need a replacement receipt. |

| Q: Where can exhibitors pick up badges and exhibit information? |
| | Exhibitors can pick up their registration materials and badges from the EXHIBIT REGISTRATION area located in Level One lobby of the Moscone West Convention Center. |

| Q: Where can exhibitors correct or replace their badges? |
| | Exhibitors can have their badge(s) corrected or have a new one printed in the Exhibit Registration area located in Level One lobby of the Moscone West Convention Center. There is a fee of $150 to reprint or replace any Exhibitor badge. |
|
| Q: How many complimentary badges is my company allotted? |
| | Each exhibiting company receives 5 badges for every 10'x10'
booth space purchased. Complimentary badges that are not reserved on or
before September 17, 2004 will be forfeited. No complimentary badges will be issue
on-site. All registrations over the complimentary allotment, made by September 17th,
are $100 each.

| Q: What if I don’t know all the names of the individuals working in my booth prior to the Advanced Registration deadline of September 17, 2004? |
| | Additions or changes made after September 17, 2004 must be made on-site. All badges issued on-site (including replacement badges) are $150 each regardless of the number of personnel that are pre-registered. |
|
| Q: How can I receive Opening Reception tickets? |
| | Exhibitors staying within the CNS Housing Block will
receive a maximum of two complimentary tickets to the Opening Reception on
Sunday evening for each 10x10 booth contracted (up to a maximum of 24 tickets).
Exhibitors staying outside the CNS Housing Block may purchase tickets to the
Opening Reception on-site on a first-come, first-served basis. Ticket prices are $100
Each. Dress for the Opening Reception is business attire.

| | Yes, Freeman Decorating Company will accept crated, boxed or skidded materials beginning September 7, 2004 at the address
listed below. To avoid additional charges, materials must arrive by Friday, October 8, 2004.

| Q: What time can I enter the Exhibit Hall? |
| | Exhibitor personnel will be permitted on the exhibit floor at 8:00a.m. on installation/dismantle and show days and must vacate one half hour after the daily closing of the Exhibit Hall. |

| Q: What can I do if I want to set up a separate meeting for my customers or sales staff? |
| | A Function Space request form is included in the Exhibitor Service Kit.
The deadline to request space is August 25, 2004. Function space is limited and will
be assigned on a first-come, first served basis. Function space held by the CNS will
be released only to companies exhibiting at the 2004 CNS Annual Meeting. CNS
will approve Function Requests via fax. After you receive your faxed approval, you
may work directly with the hotel to arrange your event.

| Q: How do I get electric, furniture, housing, floral, cleaning Internet service, etc., for my booth? |
| | The exhibitor Services Kit is available online through the CNS General Contractor,
Freeman Decorating. Freeman emailed all CNS Exhibitors a personalized link to the online kit earlier this year.
If you did not receive this information or would like it resent, please send a request to info@1cns.org or call 847 240 2500.
All forms should be completed in advance of the meeting to ensure that adequate labor,
equipment, etc., is available for an easy setup. Download this Exhibitor Checklist to
keep track of important deadlines.

| Q: Can I hang signs and banners in my booth? |
| | Banners or other identification suspended or otherwise separated from the booth itself are not permitted. See page 8 of the Exhibitor Prospectus for details. |

| Q: What can I do to increase my booth traffic before and after the Annual Meeting? |
| | Send direct mail announcements to existing customers and attendees who have pre-registered
for the CNS Annual Meeting. A pre-registered mailing list will be available for purchase beginning September 16, 2004. A post-show list will be available 2-3 weeks after the Annual Meeting.
Note: You must be a 2004 CNS Exhibitor to purchase these lists. An order form is
provided in the Exhibitor Services Kit.

| Q: How do I attract more attention during the meeting? |
| | Use the hotel room drop service to deliver your promotional materials to attendees
staying at the official CNS hotels. You may also conduct a Raffle or Drawing from
within your booth (Requests for in-booth contests, raffles and giveaway items are due
to CNS on September 10, 2004. Contact CNS for more details.

| Q: Where is the CNS Spouse Hospitality Suite located? |
| | The Spouse Hospitality Suite is located in the Cityscape Restaurant at the Hilton San Francisco. A continental breakfast is available Monday – Wednesday from 7:00 am – 10:30 am. Admittance to the Spouse Hospitality Suite is by Spouse/Guest or Nurse/Spouse badge only. |

| Q: What is the climate in San Francisco? |
| | San Francisco enjoys mild weather year-round. October temperatures range from a high of 68 degrees to a low of 54 degrees. Visitors are most comfortable with a light jacket, coat or in a suit. An all-weather coat will take the chill off cool evenings. |

| Q: Where is the Opening Reception? |
| | The Opening Reception is in the Grand Ballroom of the Hilton San Francisco from 6:30 pm to 8:30 pm. Each annual meeting registrant will receive one complimentary ticket to the Opening Reception. Additional tickets can be purchased for $100 each. |

| Q: Who is the 2004 CNS Honored Guest? |
| | Dr. Arnold H. Menezes is this year’s Honored Guest. He will be speaking at the General Scientific Sessions throughout the week. Dr. Menezes will also be speaking at the Practical Courses PC06 and PC16 and Luncheon Seminars M01 and T17. |

| Q: Who is the Walter E. Dandy Orator? |
| | The CNS is pleased and honored to welcome President Mikhail Gorbachev, one of the twentieth century’s most pivotal leaders, as the Fifth Annual Walter E. Dandy Orator. President Gorbachev will speak on Tuesday, October 19th, 11:10 am at the General Scientific Session. |

| Q: Who is the Special Lecturer on Creativity and Innovation? |
| | Referred to as “the world’s greatest airplane designer,” Burt Rutan is this year’s CNS Special Lecturer. Mr. Rutan will speak on Wednesday, October 20th, 8:15 am at the General Scientific Session. |

| Q: Where can I pick up or leave a message? |
| | The message center is located in the CNS Registration Area - Level One Lobby at the Moscone Convention Center and is open during registration hours. |

| Q: When can I receive my Record of CME (also called Certificate of Attendance) |
| | All tickets for “ticketed” events are collected during the meeting and shipped back to the CNS Headquarters’ Office. At that time, tickets are scanned and CME is recorded. All medical registrants will receive 25 Category 1 hours of CME. Additional CME is given if tickets are “handed in” for practical courses and luncheon seminars. Certificates of attendance are mailed approximately six weeks after the meeting.
Please note our new procedures for CME. Each medical registrant will be given a tracking form in their registration packet listing all CME activities. Prior to their departure from the Annual Meeting, please submit your form indicating the number of hours of CME activity that you actually attended. You can submit your form in the Registration area at Moscone West (through Wednesday, October 20) or at the Hilton San Francisco (Thursday, October 21). NO CME certificates will be issued unless a CME tracking is submitted at the Annual Meeting or faxed to CNS Headquarters to 847 240 0804. |

| Q: Can I get my friend/child/medical technician into the exhibit hall without registering him/her? |
| | No. Everyone who enters the exhibit hall must be a registrant of the meeting. There are no one-day registrations and all guests must pay the full guest fee of $115. Due to safety concerns, children under the age of 18, are not permitted in the Exhibit Hall at any time even if they are registered. |

| Q: Is there somewhere I can check my email while attending the meeting? |
| | Yes. E-mail Cafés will be available in the CNS Member Services Center in Booth #337 and throughout the Exhibit Hall. Please be sure to have your email account username, password and the mail server name. |

| Q: Where will future CNS Annual Meetings be held? |
| | Future CNS Annual Meetings will be held in the following locations: Boston, MA from October 8 – 13, 2005 Chicago, IL from October 7 – 12, 2006 San Diego, CA from September 15 – 20, 2007 |

| | The San Francisco Airport is 13 miles from downtown San Francisco and travel can range from 15 minutes to 45 minutes. The Oakland airport is 19 miles from downtown and travel times can range from 25 minutes to an hour. |

| Q: How much is a taxi from the airport to downtown San Francisco? |
| | Taxis are readily available outside the baggage claim area at the San Francisco International Airport. The fare will be approximately $37 to the downtown area. |

| Q: Who should attend the CNS Annual Meeting? |
| | Neurological surgeons, neurosurgery nurses, nurse practitioners, physician assistants, orthopedic surgeons, primary care physicians, gerontologists, radiologists, hospital administrators, oncologists, neurologists, pediatricians, physiatrists, and infectious disease specialists are welcome and encouraged to attend the 2004 CNS Annual Meeting. |
